Title: SNAKE BITE ENVENOMATION DURING PREGNANCY

Introduction:- Snakebite is a rare event during pregnancy but a large series of hospital admissions from India reported 1% rate of snakebite in pregnancy. Aims & Objectives: To review a case of snakebite envenomation during pregnancy. Material & methods: Hospital based study done in Dept of OBGY, KMC, Katihar, Bihar. Case summary:- A 26yrs old female G2P1L1 at 34wks of gestation was referred from Manihari Hospital to our facility with one day history of snakebite, pain abdomen and difficulty breathing since 1 day. Patient gave history of snakebite on right hand. O/E her right hand was swollen with discolouration of surrounding skin. Patient had already received anti snake venom at Manihari Hospital. Patient was an unbooked case with no antenatal complication till present. O/E, general condition was fair, pallor ++, PR 106bpm, BP 100/70mmhg. Spo2 97% on room air. CVS S1S2 heard and normal, RR was 22breaths/min and chest was clear. Result: On P/A examination, uterus corresponded to 34wks, mild contraction present and FHR was 140bpm. Presentation was cephalic. On P/V examination cervix was 3cms dilated and 30% effaced. Foley?s insitu maintaining u/o of 70ml/hr which was blood stained. On admission, all investigations were within normal limits. Patient was shifted to labour room. Intrapartum period was uneventful and delivered normally. Baby cried immediately after birth. 1 pint PCV and 2 FFP was transfused post delivery. Conclusion: Snakebite is uncommon in pregnancy, when it occurs, it?s associated with feto-maternal complications depending upon the degree of envenomation. The maternal and obstetrics complications observed were various like obstetric hemorrhage, preterm labour and delivery, DIC, hypovolemic shock. While fetal and neonatal complications included tachycardia, prematurity, jaundice, sepsis. Prompt treatment with antivenom and adequate blood products is vital in saving the mother. Timely delivery of the fetus needs to be considered as well.
