Title: TORSION OF LARGE OVARIAN CYST DURING PREGNANCY- A CASE REPORT

CASE REPORT : A 22 year old Primigravida with 14 weeks of gestation was registered at Kamineni hospital, Hyderabad with NT scan incidentally showing a cyst of ?ovarian origin of size 17x10 cm extending from epigastric region to pelvis. 2 days later, she came with complaints of pain abdomen and 2 episodes of vomitings and under Spinal Anesthesia, underwent Emergency Open laparotomy with Left Ovarian Cystectomy with Detorsion. Intraoperatively, a 20x20 cm Left ovarian cyst was noted with two turns of torsion, Cyst wall punctured and straw coloured clear liquid of around 1300 cc aspirated, cyst wall excision done and redundant ovarian tissue approximated, Left ovary retained. Postoperative period was uneventful. Histopathological examination of the cyst revealed it to be a Benign Serous Cystadenoma. Her further antenatal visits were uneventful and she now corresponds to a gestational age of 25 weeks DISCUSSION: Ovarian torsion is a phenomenon describing a complete or partial rotation of the adnexa around its own vascular axis or pedicle. The precise etiology is unknown, however, the predisposing factors are moderate size cyst, free mobility and a long pedicle. . During pregnancy, there happens to be a 5 fold increased risk of ovarian torsion, with an incidence accounting up to an approximate 5 per 10,000 pregnancies. The incidence is greater at gestational age of about 10?17 weeks with ovarian masses greater than 4 cm. In Conclusion, Ovarian torsion is an emergency gynaecological surgery that needs to be performed irrespective of gestational age
