Introduction Placenta percreta, the most severe form of placenta accreta, is a rare pregnancy disorder in which the placenta penetrates the uterine myometrium and serosa and can invade the surrounding organs. It is a potentially life threatening condition with risk of severe maternal morbidity and mortality Aims & Objectives we present the case study of 27 year old primigravigda 38week nd 0 days pregnancy presenting as breech presentation with PROM in Labour which turned out to be a unsuspected case of perforating placenta percreta at the time of emergency c-section Material & Methods descriptive case study Results in the purview of excessive bleeding during failed attempt to separate placenta and salvage uterus ,emergency ceseraian hysterectomy was proceeeded with .the patient was symptomatically managed and is not associated with adverse fetal and maternal outcome Conclusions prenatally using various diagnostic modalities like MRI and sonography the diagnosis of placenta accreta spectrum could be made which will require multidisciplinary management in tertiary care hospital to improve outcome .in such un diagnosed case at the time delivery a team of anesthesiologist, obstetrician, urologist, neonatologist, and blood bank officer is needed for successful management of these patients. Caesarean hysterectomy is probably the preferable treatment and conservative management should only be used in highly selected cases and in places where such facilities are available
