Abstract :
Introduction: Chronic Kidney Disease affects 5% of female child bearing age, having adverse maternal and fetal outcome. High renal burden as a results of pregnancy may lead to deterioration of renal function. And increasing number of women with CKD stage3-4 have a strong desire to conceive and both obstetrician and nephrologist are faced challenges in term of their treatment.
Aims and Objectives: The aim of managing CKD during pregnancy is to ensure good maternal and fetal outcome. Identify the early signs and symptoms of chronic kidney disease to facilitate prompt diagnosis and intervention.
Materials and Methods: A 21 years primigravida at 37+2weeks presented in our emergency labor room with breathlessness, anasarca and anaemia in labor. USG report and Blood investigation suggestive of Chronic parenchymal disease. She was managed with both Obstetrician and Nephrologist conservatively and planned for haemo-dialysis.
Results: She delivered a vaginally 2.5 kg male baby and developed pulmonary oedema in 3rd stage of labor and managed conservatively. After stabilizing the patient, she was taken for haemo-dialysis. She was undergone 4 haemo-dialysis and creatinine raised down 2.1 from 7.6 mg/dl. She become haemodynamically stable.
Conclusion: CKD is a challenge of pregnancy from early stages. Strict follow up is needed for CKD patient even when there is normal renal function. CKD is a growing health care problem, only recently acknowledged in it's full dimensions.
