Abstract :
Introduction Subinvolution of placental site(SPS) is characterised by the abnormal persistence of large, dilated, superficially modified spiral arteries in the absence of retained products of conception. Aims and objective - To evaluate the cause of intractable vaginal bleeding post abortion as diagnosis of SPS is? often under recognised by obstetricians due to the absence of specific diagnostic criteria. Material and methods Case report A 32 year old with a parity score of P2L2A2, presented with complains of intermenstrual bleeding since 8 days, passage of clots and pain abdomen.Dilatation and curettage done in view of missed abortion 2 weeks ago. Patient presented with stable vitals, abdomen examination was normal , speculum and vaginal examination revealed fresh bleeding and normal sized uterus. Blood investigation showed Hemoglobin of 6.2g%, beta HcG of <0.1 IU/ml. Ultrasound showed AV malformation and fibroid uterus. Results Medical line of management with Injection Tranexemic acid, blood products and antibiotics were given . As patient?s clinical condition deteriorated the decision of total abdominal hysterectomy with bilateral salpingectomy was taken. Intraoperatively, uterus was fragile with normal size. Hematometra noted along with hemoperitoneum. Tubes and ovaries were normal. Her histopathology report showed subinvolution of placental site Conclusion SPS although rare can be a life threatening condition due to its difficultly in diagnosing it clinically and on ultrasound as it closely resembles AV malformation.
