Introduction: Congenital malformations of the female genital tract are defined as deviations from normal anatomy resulting from embryological maldevelopment of the Mullerian or paramesonephric ducts. Aims and Objectives: Aim : To present a case of term pregnancy in a rare Mullerian anomaly bicorporeal septate uterus, double normal cervix and longitudinal non obstructing vaginal septum. Setting : Katihar Medical College, Katihar Materials and methods: Case summary: A 21-year-old primigravida presented to us at 37+3 weeks of gestation with complains of draining liquor per vaginum and on and off labour pain. Abdominal examination was suggestive of breech presentation with mild uterine contractions. On vaginal examination, a longitudinal non-obstructing vaginal septum in the middle was noted. A cervical septum with two separate cervices was also noted. Active leaking of clear liquor was noted from the right external os, and the right cervix was 2 cm dilated. Results: Patient was taken up for emergency cesarean i/v/o breech presentation in latent phase of labor with prolonged rupture of membranes. Intra-operatively, liquor was found to be clear and adequate. A live active small-for-date (SFD) female baby was delivered by breech extraction. Two distinct and separate uterine cavities with a complete septum between was noted. Double normal cervix was confirmed. Within the uterine body and cervix, no communication between the two cavities found. Conclusion: Complete septate uterus has the worst reproductive and pregnancy outcomes compared with another malformed uterus. Our patient had an uneventful pregnancy up to term except for SFD baby; while malpresentation is a proven complication of uterine anomaly.
