Abstract Reverse Takotsubo cardiomyopathy (RTC) is a rare, stress-induced cardiomyopathy. Though typically seen in postmenopausal women, RTC is increasingly recognized in the postpartum period, often triggered by the physical and emotional stress surrounding childbirth and surgery. We present two unique cases of RTC in postpartum women, highlighting the diverse ways in which this condition can manifest. The first case involves a 32-year-old woman who, 18 days after an uncomplicated vaginal delivery and bowel resection for intestinal obstruction, suddenly collapsed with severe chest pain. Echocardiography revealed the classic signs of RTC, while coronary angiography excluded other cardiac causes. After supportive care, her heart function returned to normal. The second case is that of a 28-year-old woman with Primi/ term/ severe preeclampsia, who developed RTC after an emergency cesarean section. Postoperatively, she experienced chest pain and shortness of breath, and echocardiographic findings confirmed RTC. With appropriate treatment, her symptoms improved, and she completely recovered. Both cases highlights the need for clinicians to consider RTC in postpartum women presenting with acute cardiac symptoms, particularly after significant obstetric or surgical stress. Early diagnosis and treatment are crucial, as RTC is typically reversible with the right care. This case report emphasises the importance of clinical vigilance and contribute to the growing understanding of this rare but potentially serious condition in postpartum women.
