Title: SURGICALLY CORRECTED HEART DISEASE IN PREGNANCY: AN EXPERIENCE AT TERTIARY CARE CENTRE

INTRODUCTION : Heart disease and its complications are an important cause of maternal mortality and morbidity both in antepartum and postpartum period. The overall incidence of heart disease in pregnancy is <1%. With advancement in medical field, surgical correction during pregnancy markedly improves the NYHA class status in medically refractory cases and it improves the maternal and fetal outcome AIMS & OBJECTIVES: To analyse maternal and fetal outcome and prognosis related to cardiac surgery performed prenatal and during pregnancy. MATERIALS AND METHODS: It was a retrospective study conducted in the Department of Obstetrics and Gynaecology, ABVIMS and Dr RML hospital, New Delhi. Twenty-nine women with history of cardiac surgery prenatal or antenatal were enrolled in the study and their fetomaternal outcome were noted. RESULTS: The principal cause of cardiac lesion was Rheumatic heart disease (RHD) (89.65 %) while congenital heart disease was seen in 10.34%. PTMC was the most common heart surgery performed in these cases followed by MVR. Incidence of preterm deliveries was 41.37%. Vaginal delivery was seen in 51.72% cases, while 44.82% underwent CS. NICU admission was required in 17.24% cases. CONCLUSION: Advancements in both cardiac and obstetric care have made it possible for many women with surgically corrected heart disease to undergo pregnancy with good outcomes, but it requires highly individualized management with a multidisciplinary approach.
