Abstract :
Introduction An unusual case of monochorionic diamniotic twin pregnancy with TRAP sequence managed with minimally invasive fetal surgery. Aims & Objectives MCDA with TRAP sequence is rare presentation with prevalence of 0.3-1 % means 1:35000 births To achieve optimum outcome multidisciplinary approach is mandated of which R.F. ABLATION is studied in our index case. Method & Material Primigravida, infertility conception was diagnosed at 13 weeks with MCDA twins with TRAP sequence. Serial USG monitoring showed increase in volume of acardiac fetus to 25 cc from 2.8cc Decision of R.F. ABLATION with risk of miscarriage explained. Option of MTP given. With high risk consent R.F. ABLATION done at 17 weeks GA. Post procedure, acardiac twin perfusion absent and donor twin showed good cardiac activity on USG. Regular patient monitoring with USG shows normal parameters for mother & fetus. Results After conservative surgery donor fetus showing proportionate growth & normal color Doppler studies and acardiac twin perfusion absent till date. Conclusion With multidisciplinary TEAM approach and vigilant monitoring, such complicated case can be managed conservatively with good outcome even at periphery.
