Abstract :
INTRODUCTION: Filled with inquisitiveness and intense wish to explore orifices and sometimes sexual fantasy, children often land up with foreign bodies in various tracts. Usually, the foreign body insertion is disclosed to the parent prompting a visit to healthcare facility for removal. Alternatively, the object may go undisclosed due to embarrassment or memory lapses, or if the young child is nonverbal resulting in its retention for extended periods producing vaginal discharge, spotting or even fistulas. CASE: We present a case of a 12-year-old girl with persistent malodorous vaginal discharge over about eight months. On initial examination, wart, polypoidal growth was seen in vagina with foul smelling discharge. Histopathology of punch biopsy specimen taken from the growth was reported as fibro- epithelial polyp. With persistent denial by patient and parents of foreign body insertion by self or by someone else and initial negative hysteroscopy, this case became a diagnostic dilemma. But ultrasound and magnetic resonance imaging were consistent with their diagnoses of foreign body and played a crucial role in delineating its location. Patient was re-examined under general anesthesia and when the polypoidal tissue was cut with pencil cautery, a hole got identified inside which hysteroscope was inserted and the foreign body started to become visible.?The foreign body, a lipstick cap was then extracted out. It was retained in vagina for over 2 years, resulting in growth of a granulation tissue all around it making it incarcerated deep within the posterior fornix between uterine body and the posterior vaginal wall, making it invisible on initial examination under sedation. CONCLUSION: This case highlights that evaluation of persistent symptoms of vaginal discharge in a young girl must include a comprehensive history, thorough physical examination and focused imaging to look for any retained vaginal foreign bodies. With hysteroscopy and vaginoscopy even deep-seated foreign bodies can be extracted safely. Sex education and parental vigilance is imperative especially when there are growing toddlers and adolescents to avoid foreign bodies and their adverse sequelae.
