Abstract :
Introduction: Pseudomyxoma peritonei is a rare complication of mucinous neoplasms of ovary, characterized by mucinous ascites and jelly like deposits spread across peritoneum. While most pseudomyxoma peritonei cases stem from appendiceal neoplasms ( 80-90%) , the same from ovarian neoplasm are comparatively rare ( 6-7 % ) Aims and objectives: This is a case report of a benign ovarian neoplasm associated with pseudomyxoma peritonei . The objectives are to review the unusual presentation, diagnostic and management challenges. Materials and Methods: A 55 years old P2L2 post menopausal woman came with complaints of abdominal distension since the past 3 months. On examination a vague mass was felt in the Right lower abdomen around 10x 12 cms . Imaging revealed predominantly cystic lesion with thick internal echoes within the pelvis with superior extent into the umbilicus with internal vascularity. There was no preoperative pointers towards mucinous ascites in clinical evaluation or imaging. A preoperative diagnosis of Benign ovarian neoplasm was made. Result : Patient underwent surgical exploration through a midline laparotomy incision. Intraoperative findings : Mucinous fluid with jelly like deposits present all over the peritoneal cavity. Right ovarian mass present (15x18 cm) . Total Abdominal Hysterectomy with Bilateral Salphingo-oopherectomy with infracolic omentectomy, bilateral pelvic lymph node sampling and appendectomy was done. After an uneventful post operative period, patient was discharged on POD- 8. Patient is healthy on follow up Histopathology : Right ovarian Mucinous Cystadenoma; no omental deposits; no nodal involvement. Appendix : Enterobius vermicularis infestation with no signs of atypia / malignancy. Conclusion While most cases of Pseudomyxoma Peritonei stem from mucinous adenocarcinoma of appendix ( 80-90%) , the same of ovarian origin is a rare occurrence. Preoperative diagnosis is difficult , thus necessitating astute intraoperative decisions .Pseudomyxoma peritonei can make management of these neoplastic lesions challenging, at times necessitating complex and technically challenging interventions such as Cyto Reductive Surgery ? Hyperthermic Intra Peritoneal Chemotherapy ( CRS ? HIPEC) and peritonectomy. Prompt surgical interventions with appropriate adjuvant treatment is paramount for optimal outcome.
