In women of reproductive age, ovarian masses can be benign or cancerous, thus diagnosis and treatment must be done carefully. A 42-year-old woman who presented with a palpable lower abdominal mass, pelvic pain, and occasional bloating is the subject of this report. She had a regular menstrual cycle and no notable medical or familial history of ovarian cancer. A hard, movable mass in the lower abdomen was verified by physical examination. A 27-cm complicated, unilocular cystic mass in the left ovary with both solid and cystic components was discovered by pelvic ultrasonography, which raised the possibility of a malignancy. The results of laboratory tests, such as CA-125, were within normal ranges, indicating a decreased cancer risk. An MRI discovered a big unilocular tumour with several non-enhancing solid nodules. Nevertheless, surgery was deemed suitable due to the bulk features and the patient's clinical presentation. Following an oophorectomy, the patient's intraoperative findings revealed a well-encapsulated ovarian tumour free of local invasion. A mucinous cystadenoma without any malignant characteristics was established by histopathological investigation. The patient recovered from the procedure without any problems and was sent home with instructions for regular follow-up. This example demonstrates the diagnostic factors to be taken into account when a young woman has a palpable ovarian tumour.
