Title: MULTIPLE PRIMARY CANCER INVOLVING ENDOMETRIUM AND BREAST -A RARE CASE REPORT

INTRODUCTION Breast carcinoma is most frequently diagnosed malignancy worldwide . Endometrial carcinoma are considered as the commonest type of gynecological cancer mostly affecting postmenopausal women. Multiple primary malignancy is the primary malignancy with different histology to two or more distinct parts of the body . Multiple primary cancers are synchronous when new malignancy is diagnosed within 6 months and metachronous when diagnosed after 6 months. The possibility of synchronous endometrial and breast cancer in a single person is very much low with an incidence less than 0.5%. This association may be because of common etiologies like genetic predisposition, hormonal risk factors or treatment related factors and obesity. CASE PRESENTATION A 68 year old women presented with complaints of Left breast lump of 3 weeks duration. She was evaluated and diagnosed to have invasive breast carcinoma non specific type stage I(T1 CN0M0), ER, PR strong positive, Her2 (0) k16-7 -19%. On evaluation found to have thickened endometrium with internal vascularity in USG. Endometrial sampling done found to have adenocarcinoma endometrium. MRI abdomen and pelvis done, showed carcinoma endometrium FIGO stage II. Decided to go for primary surgeries for both malignancies.- Breast conservative surgery-wide excision of left breast lump+ matrix rotation oncoplasty + sentinel lymph node biopsy for breast and Modified radical hysterectomy + lower para aortic lymph node dissection. Post operatively locoregional EBRT and vaginal brachytherapy given. CONCLUSION As synchronous breast and endometrial cancer is a rare event, it should be managed under a multidisciplinary team, and it should be individualized according to the patient. Surgery for breast and endometrial cancer improves local control and survival and increases the chance of curability. In case of endometrial cancer radiotherapy and brachytherapy increase the local control and prognosis and a complete response.
