Abstract :
A 45 years old P2L2A1 peri menopausal woman came with complaints of bleeding per vaginum since 2 weeks associated with passage of clots and on and off constipation. On examination uterus ~14 weeks, firm to soft in consistency. Imaging done by MRI pelvis with contrast revealed a lobulated heterogenous lesion-7.7 X 6.9 X 10.6 cm involving uterine fundus and upper right wall of uterine body with multiseptated cystic/ necrotic component ( 8.5 X 7.5 cm) projecting outwards towards right side and a provisional diagnosis of ?uterine fibroid with degeneration was made. Patient was taken up for Total Abdominal Hysterectomy with Bilateral Salpingoopherectomy however intraoperatively dense adhesions were noted with bowel adherent to the posterior wall of the uterus with uterus being 12 X10 cm in size and cystic in nature and hence surgical oncology assistance was requested. Additional Infra colic omentectomy with bilateral pelvic lymph node sampling was done and sample was sent for HPE. After an uneventful peri- and post operative period, patient was discharged on POD-9. Hisopathology revealed : High Grade Endometrial Stromal Sarcoma with IHC positive markers. Patient was followed up regularly following the primary treatment of surgery and supplemented with adjuvant chemotherapy.
