BACKGROUND: Autoamputation of adnexa is a rare finding caused by structural injuries usually due to ovarian torsion. Diagnosis for torsion can be achieved through laparoscopy, with gold standard management to include laparoscopic detorsion and cystectomy. CASE REPORT: A 28 year old G2P1L1 previous IVF conception and delivered at 30 weeks presented with cyst in second pregnancy. The patient was posted for cervical encirclage in view of previous preterm delivery and along with it laparoscopy was done at 12 weeks of gestation where right ovarian torsion was noted which had undergone auto amputation. As cervical length was good Shirodkar?s encirclage was done. Patient continued the pregnancy till term and had opted for elective caesarean section. Intraoperatively necrotic ovaries were seen and removed and sent for HPR which showed old haemorrhagic cyst. CONCLUSION: Spontaneous ovarian torsion with autoamputation during the first trimester of pregnancy is a rare but critical condition that requires prompt recognition and intervention. If untreated, it can lead to ovarian necrosis, loss of the affected ovary, and potentially severe complications like infection or sepsis, which can jeopardize both maternal and fetal health. Early diagnosis via ultrasound and timely surgical management, whether through untwisting or removal of the necrotic ovary, significantly improves the outcome.
