Abstract :
INTRODUCTION PPH is the leading cause of maternal death in india,most common cause being the uterine atony. Heat stable carbetocin is long acting syn.octapeptide analogue of oxytocin with agonist properties. AIM&OBJECTIVE To observe the trend of blood loss,safety,efficacy and side effects between oxytocin and carbetocin. MATERIAL & METHOD-Study Design ?retrospective observational study. Conducted at peoples medical college ,Bhopal over a period of 1 year. Sample size ? 115 in oxytocin group 137 in carbetocin group Data collection- ? All willing full term vaginal delivery patient getting delivered in hospital were given oxytocin for 6 months followed by carbetocin for next 6 months. ? Post delivery assessment was done and were observed for impact on amount of blood loss, haemoglobin levels at the time of admission and after 48hrs of delivery, need for additional uterotonics if blood loss exceeds 500cc, need for blood transfusion, adverse effects like headache, nausea, vomiting, sweating, palpitation, fever. ? Oxytocin will be given at 10 IU iv in 500ml RL and 10 IU i/m ? Carbetocin given as a single dose of 100 mcg im. ? The blood loss will be calculated by visual estimation. ? Visual estimation is made by pictorial blood assessment. RESULT - PPH was noted in 7 patient (6%) in oxytocin group wheras 2 patients (1.5%) had PPH in carbetocin group. 3 patient (2.6%) required blood transfusion in oxytocin group and 0 pts.carbetocin group. Avg. blood loss in oxytocin group was 332.5ml and in carbetocin it was 298ml 17 (14.7%)need uterotonics in oxytocin group and 9 (6.7%) in carbetocin group CONCLUSION-heat stable carbetocin is expected to reduce no.of pph events at reduced cost to public healthcare adoption of carbetocin in AMTSL could advance efforts to improve maternal health goals and increase efficiency of its public health expenditure.
