Abstract :
Introduction
Acute pancreatitis is a common acute medical condition requiring emergent care . Acute pancreatitis during pregnancy is a rare but severe clinical problem causing significant maternal and perinatal mortality and morbidity .occurring in 1 in 1000 to 1 in 12000 pregnancies. It is more common in multigravidas (75%) occurring commonly in the third trimester (50%) and postpartum period (38%) and is relatively uncommon in the first trimester (12%)
CASE
32 year old gravida 2 para1 living 1 with 31 weeks 5 days of period of gestation with k/c/o hypothyroidism . came with complaints of upper pain abdomen radiating to back, increases on lying down and reduced on sitting , associated with 2 episodes of vomiting , non projectile and bilious, non blood stained. ultrasound showed head of pancrease bulky, and hetero echoic in echo textures s/o acute pancreatitis ,blood investigations showed raised lipase and amylase 3 times the normal. Diagnosed with acute pancreatitis. .she had history of acute pancreatitis in her previous pregnancy at third trimester managed conservatively, the cause was not elucidated. patient was managed conservatively by multidisciplinary approach including gastroenterologist, pain and palliative medicine . patient underwent emergency LSCS in the view of fetal distress at 32 weeks 6 days of gestation. patient improved symptomatically and repeat scan showed no peripancreatic collection/necrosis . patient discharged on pod 6.
Conclusion
Pancreatitis in pregnant women, especially acute pancreatitis, is a challenge for both obstetricians and gastroenterologists. Acute pancreatitis can be potentially life-threatening to both the mother and the fetus. Therefore, risk factors such as gallstones and hypertriglyceridemia should be diagnosed early in pregnancy. A multidisciplinary approach, including gastroenterologist, surgical and obstetric care is essential to manage it effectively.
